A care plan is a document that contains detailed information, such as our clients' level of support required, preferences, likes/dislikes, hopes and goals for the future. This enables our clients to be the focal point in their care package, providing a unique plan of action that will ensure quality care is always provided and progression takes place.
Not only do we provide the care plan, but we regularly review them with our clients, so that their care can be altered quickly and efficiently should their needs change.
